Thank you for choosing the GACS Monthly Online UST Walk-Through Inspection Form. To activate access, you will need to register your location using one of the FOUR official payment registration options below.
There are TWO GACS MEMBER registration options and TWO NON-MEMBER registration options.

GACS MEMBER REGISTRATION OPTIONS
GACS MEMBER REGISTRATION OPTIONS AND DIRECTIONS:
1) You MUST log in to your membership portal for the GACS membership rate. Click on the member option below and the system should prompt you to log in.
2) You may choose either the pay monthly ($15 per month) or pay annually ($180 per year) option.
3) Once you complete the initial payment process, you will be contacted by Kurt Winters of TankForms who will complete your onboarding process or you can reach him at 817-296-4466 or [email protected] should you have questions. After the initial registration and onboarding you will have access to the online form/app and your card will automatically be billed each month until you OPT OUT of the forms.
4) This sign-up is good for ONE location. If you want to sign up for multiple (5 or more) locations, please contact Kurt directly at 817-296-4466 or kurt@tankforms.
Special onboarding and monthly pricing packages are available for multiple (5 or more) locations.
NON-MEMBER REGISTRATION OPTIONS
NON-MEMBER REGISTRATION OPTIONS AND DIRECTIONS:
